We have identified an issue where some Tines forms will have the title and description fields repeated. The issue started at 10:30 UTC on Tuesday 22nd of November. Impact: There was NO impact on running stories and there was NO data loss. Status: The root cause has been addressed, and the repeated fields are being removed. There is no user action required.
